Registration 2025-26 School Year

Open registration for St. Agnes Cathedral School will occur in-person on
Tuesday, January 28th from 9:30-10:30 A.M. and 7:00 -8:00 P.M.
Wednesday, January 29th from 9:30-10:30 A.M.

1. Registration Letter Parental Confirmation
2. Registration Form 
3. Pertinent Records Release Form
4. A current Immunization Form completed by child's physician
5. Birth and Baptismal certificates 
6. St. Agnes Cathedral Parish offertory account number or proof of contributions from another parish where registered  
7. A $150 non-refundable registration fee for grades 1 - 7
8. A $300 non-refundable registration fee for Kindergarten

The following Health Forms (except Health Information Letter) are required to be completed and sent to the School Nurse, Mrs. Kristine Keating, no later than March 21, 2025.
