Registration 2023-24 School Year

Open registration for St. Agnes Cathedral School will occur in-person on
January 31st and February 1st.
January 31st from 9:30-10:30 A.M. and 7:00 -8:00 P.M.
February 1st from 9:30-10:30 A.M.

1. Registration Letter Parental Confirmation
2. Registration Form (Printed e-Version or Paper Version)
3. Pertinent Records Release Form
4. A current Immunization Form completed by child's physician
5. Birth and Baptismal certificates 
6. St. Agnes Cathedral Parish offertory account number or proof of contributions from another parish where registered  
7. A $100 non-refundable registration fee for grades 1 - 7
8. A $300 non-refundable registration fee for Kindergarten

The following Health Forms (except Health Information Letter) are required to be completed and sent to the School Nurse, Mrs. Kristine Keating, no later than March 25, 2022.
