Special Services

Health Services 

Rockville Centre School District #21 provides the services of a nurse five days a week. Children on designated grade levels are examined annually for vision, hear-ing, and scoliosis. 

A. Physical Examination 

New York State Law mandates two types of health appraisals. The first is a complete physical examination that is required in K, 2nd, 4th, and 7th grade. The second is an annual assessment for participation in interscholastic sports. All exams are acceptable for 12 months from the date of the examination. No exceptions will be made. 

B. Immunization 

In accordance with the Amendments to the New York State Public Health Law and Regulations, a child will NOT be admitted to school without proof of the following immunizations: 

If a child becomes ill or has an accident during the school day, every attempt will be made to contact the parent(s) or legal guardian(s). Emergency cards must be on file in the Health Office and updated as needed. 

D. Medication 

State law prohibits the dispensing of medication, including any nonprescription drugs such as Tylenol, Advil, etc, EXCEPT by a written request from the parent/parent relation and a written order from a licensed New York State healthcare provider (physician, nurse practitioner, or physician assistant). The med-ication forms are available in the nurses office and on the school website, under Health Care. All medication must be submitted to the nurse in the original container. NO EXCEPTIONS will be made.

E. Medical Excuse for Physical Education 

If your child cannot participate in gym class, a written excuse from a physician must be submitted to the health office. Medical clearance must also be sub mitted when the child can be readmitted to physical education classes. Students that are not able to participate in physical education classes are not permitted to attend outdoor recess, try out or participate in interscholastic sports 

F. Reportable Conditions 

All incidents of injuries, serious illnesses, surgery, accidents and communicable diseases must be reported to the health office. Communicable diseases that must be reported are: chickenpox, conjunctivitis, fifth disease, flulike illnesses, lice, mononucleosis, ringworm, strep throat and scarlet fever. 

G. Returning to Class 

If a student has had a fever, he/she must remain out of school for a period of 24 hours following the last fever. 

H. Students and parents of 6th, 7th and 8th graders, must read, sign and return the Student and Parent Acknowledgement of Receipt Form: Concussions: The Invisible Injury, during the first week of school. NO student is allowed to try out for any school sports without this form 

I. As of July 1, 2012, all parent/person in parental relation must be aware of the concussion protocol St. Agnes/RVC School district must follow as per NYS law. If your child is diagnosed with a concussion and your child is cleared to return to all activities by your attending medical care provider, he/ she must ALSO be cleared by the Rockville Centre School District Nurse Practitioner. Separate paperwork will be provided. 

Speech Correction 

Students in need of corrective speech therapy are referred to a qualified therapist who is provided by the respective Public School District. All incoming students are screened. Periodic re-evaluation ensures the continuity and quality of this service. 

Psychological Services 

Psycho-Educational services are provided for individually referred students by their respective Public School District. Screening is provided for incoming Kinder-gartners by the Rockville Centre Public School District. 

Special Education Services 

Students who are designated as Learning Disabled by a Committee for Special Education, receive instruction according to their Individualized Education Program (IEP).

Guidance 

Guidance services are available for all children in grades K-8. Referrals may be made by parents, teachers and administrators. Individual and group counseling is offered to students who experience family or personal concerns. Parents are encouraged to consult with the guidance counselor as needed. 

Academic Intervention Services 

Through the AIS programs, teachers provide reading and math instruction for qualified students.
